How do you cite songs in a wedding program?

I’m listing classical music and christian hymns in the wedding program. What is the proper way to list these songs? For example:
"Come Thou Fount of Every Blessing" Wyeth

    It’s as easy as title, lyricist, and composer:

    Come Thou Fount of Every Blessing
    John Wyeth (music), Robert Robinson (lyrics)

    You can add more information if you wish (date composed, etc), but that’s not necessary. You just want to give your guests enough information that they know what the song is.

    It is totally up to you, but here is how I do it. (I’m a church secretary).

    To be honest, it’s a wedding program….not a music recital. Sometimes I do list the composer, but other times, I don’t.

    PROCESSIONAL:
    Canon in D …………….Johann Pachalbel

    Or…sometimes I just do:

    PROCESSIONAL:
    Canon in D
